Ruhsal bozukluklar, bireylerin duygu, düşünce ve davranışlarını etkileyerek yaşam kalitesinde ciddi bozulmalara yol açan yaygın ve karmaşık sağlık sorunlarıdır. Depresyon, obsesif kompulsif bozukluk, bipolar bozukluk ve şizofreni gibi psikiyatrik tablolar bireyin psikolojik, sosyal ve mesleki işlevselliğini olumsuz yönde etkilemektedir. Ruhsal bozuklukların tedavisinde farmakolojik yaklaşımlar ve geleneksel psikoterapi yöntemleri yaygın olarak kullanılmakla birlikte, son yıllarda üçüncü dalga davranışçı terapilere olan ilgi artmıştır. Bu yaklaşımlar arasında yer alan Kabul ve Kararlılık Terapisi (KKT), bireylerin yaşadıkları psikolojik deneyimlerle mücadele etmek yerine bu deneyimleri kabul etmelerini ve kişisel değerleri doğrultusunda yaşamalarını teşvik etmektedir. KKT’nin temel amacı semptomları doğrudan ortadan kaldırmak değil, psikolojik esnekliği artırarak bireyin işlevselliğini geliştirmektir. Terapi; kabul, bilişsel ayrışma, anda kalma, bağlamsal benlik, değerlerle temas ve değer odaklı davranış olmak üzere altı temel süreç üzerine yapılandırılmıştır. Bu süreçler aracılığıyla bireyin olumsuz düşünce ve duygularla daha sağlıklı bir ilişki kurması hedeflenmektedir. Literatürde KKT’nin depresyon, obsesif kompulsif bozukluk, bipolar bozukluk, psikoz ve diğer ruhsal bozukluklarda psikolojik esnekliği artırdığı, yaşam kalitesini geliştirdiği ve belirtilerin şiddetini azalttığı bildirilmektedir. Bununla birlikte, farklı tanı gruplarında elde edilen bulguların güçlendirilmesi için daha fazla randomize kontrollü çalışmaya gereksinim duyulmaktadır. Bu derlemede KKT’nin tanımı, tarihsel gelişimi, kuramsal temelleri ve çeşitli ruhsal bozukluklardaki kullanım alanları güncel literatür doğrultusunda ele alınmıştır.
